PRIVACY AND COOKIE STATEMENT

Uw privacy wordt door ons gerespecteerd. TEC Maastricht streeft ernaar om uw privacy zo goed mogelijk te waarborgen en zal vertrouwelijk omgaan met de informatie die u bij ons aanlevert. Bij de verwerking van persoonsgegevens nemen wij de geldende wet- en regelgeving op het gebied van privacy in acht. In deze privacy- en cookieverklaring informeren wij u over de wijze waarop wij met uw gegevens omgaan.

CATEGORIES OF PERSONAL DATA

By using the website and the services available on it, you leave certain data with us. This also happens at our practice in the context of the execution of the treatment agreement. This may include personal data. We save and use only those personal data that are provided directly by you or which, when you provide them, it is clear that they will be provided to us for processing.

Depending on the service you use, we may collect the following information:

  • NAW data;

  • Gender;

  • Phone number (private and/or mobile);

  • E-mail address;

  • Date of birth;

  • Occupation/Daily Work;

  • Insurance;

  • Citizen Service Number (BSN);

  • The name of your other health care providers;

  • Reason for referral;

  • Main complaint and/or desire;

  • Data regarding your health;

  • The name of your health insurance company;

  • Time of your appointment;

  • Payment details;

  • Application details.

BASIS FOR DATA PROCESSING

We may lawfully process personal data about you only if we do so on a legal basis. We process your personal data because it is necessary for the performance of the agreement between you and us, as laid down in Article 6(1)(b) of the General Data Protection Regulation (AVG). In addition, we may process your personal data for another legitimate interest, such as informing our patients about current events or changes in our services. This basis is laid down in Article 6 paragraph 1 sub f of the AVG.

PURPOSES OF DATA PROCESSING

Personal data collected by us is used for the following purposes:

  • Creating your account to register with the practice;

  • Logging into your account;

  • Maintaining your medical records;

  • Scheduling an appointment;

  • Performing treatment;

  • Requesting and performing a Vecozo check;

  • Tracking preferences indicated by you;

  • Conducting a patient satisfaction survey periodically;

  • Sharing your data i.e. claiming through the factoring company.

  • Improving our services;

  • Performing other services requested by you;

  • Conducting an application process

DISCLOSURE OF YOUR PERSONAL DATA TO THIRD PARTIES

In principle, we only provide your personal data to third parties if you have given your consent. Data provision without your consent takes place if it is necessary to execute the agreement between you and us or a legal obligation requires us to do so.

RETENTION PERIODS

We will not retain your data longer than necessary for the purposes described in this privacy and cookie statement, unless required by law. For the retention of your medical data we use the statutory minimum retention period of 15 years from the Medical Treatment Agreement Act. In addition to the WGBO, if necessary, we also take into account retention periods arising from other legislation.

Infomedics, het factoringsbedrijf, zal de debiteurenadministratie ten behoeve van het TEC Maastricht gedurende 7 jaren bewaren in verband met wettelijke fiscale bewaartermijnen.

Bij sollicitaties die niet succesvol zijn, zal het TEC Maastricht uw sollicitatiegegevens maximaal 1 jaar bewaren.

SECURITY MEASURES

We take the storage of your data very seriously, and work continuously to ensure optimal security and processing of your data.

  • All your data is therefore stored on a modern ICT system in-house, and on Dutch soil;

  • The system is managed by a specialized healthcare IT company;

  • According to an ICT protocol developed specifically for medical care, our network is fully equipped to store personal and other sensitive data;

  • In addition, they continuously and proactively monitor and manage all relevant items related to security and continuity of the system so that it is always optimal;

  • Antivirus software is implemented in such a way that they provide the highest level of security;

  • Your data is stored redundantly, and an external, encrypted backup is made every day within the Netherlands;

  • Monitoring of antivirus, firewall, Internet traffic, hardware failure and other relevant anomalies come together in a monitoring system and oversee the whole;

  • Items of attention are automatically reported, and handled immediately by our ICT supports.

  • Het TEC Maastricht werkt met Zorgmail. Zorgmail voorziet in een veilige communicatie. Tussen Zorgmail gebruikers onderling ziet dit eruit als normale e-mail. Een ontvanger zonder Zorgmail ontvangt een link, die naar een beveiligde website leidt om de mail uit te lezen.

Furthermore, we have a secure mail system that allows us to send medical records to you or other health care provider if necessary and with your approval.

THE USE OF COOKIES

We use cookies for the functioning of our practice website. YOUR RIGHTS

Wanneer u persoonsgegevens aan ons heeft verstrekt, heeft u verschillende rechten die u kunt uitoefenen. Zo heeft u recht op inzage, rectificatie en het wissen van uw gegevens. Ook kunt u ons verzoeken om uw gegevens aan u of een andere partij over te dragen of om de gegevensverwerking te beperken. Het staat u daarnaast vrij om bezwaar te maken tegen een verwerking van uw gegevens. U kunt bovendien uw toestemming voor de gegevensverwerking altijd in te trekken.
